Registration Information
Division 1-Adult Men's Open (Top Flight) Registration Fee & Info
Entry fee charged per team of 3-5 players for Division 1-Adult Men's Open (Top Flight) and must be accompanied by your registration forms. All entry fees are non-refundable. 

Registration Fee: $99.00
REGISRATION DEADLINE: Friday, June 27, 2014
Late Registration Fee (June 28 - July 11): $150.00

Team captain will receive verification that the registration form was received along with team check-in procedures.

Fees can be paid by Visa, MasterCard or Money Order (payable to OCCUR). Personal checks are not accepted. Only accepting one money order per team. NO CASH ACCEPTED.

INCOMPLETE FORMS WILL NOT BE PROCESSED.
FORMS NOT INCLUDING FEE WILL NOT BE PROCESSED.
TEAMS WILL NOT BE REGISTED WITHOUT SIGNATURES.

Forms must be accompanied by copy of photo ID (copies of birth certificates are permitted for children without photo ID). ID will be verified at the tournament. False information will result in player and/or team disqualification without fee refund.

Divisions 2-20 (Male and Female)
NO entry fee required for Male and Female Divisions 2-12.

General Registration Information
Complete the downloadable form or online registration with information for each player on your team.

You must also submit a hard copy of your registration form which includes all appropriate signatures. Print legibly and make sure all information is filled out completely, including your team name.

Each player must sign the entry form.  Parent or guardian signatures are required if player(s) are under 18 years of age.

All forms must be received (not postmarked) by July 11, 2014. Forms received after this date will not be processed. July 11, 2014 is the final deadline for registration and this date will not be extended.
